Not directly. The style of attack that killed her was his idea, but Gale could never have known how it could be used. He'd talked traps with Beetee, and that trap was just an idea that came up. Presumably, it got passed on to Coin, and that's how she used it.

I'm with you, Callie - I think that everyone that's saying Josh will be no good based on how he looks needs to calm down a bit, and think about it. Do we really want people who look exactly like the characters, but can't act? Josh is a fantastic actor. He had his hair bleached blonde, and I reckon he looks close enough, judging by the trailer and images that have been released. So what if his eyes don't exactly match the books? Think about the Harry Potter movies. Daniel Radcliffe couldn't wear contact lenses because he reacted badly to them, so his eyes didn't match Harry's from the books. Did it matter in the end?

Yes, Prim died. Katniss was burned at the same time. At that point, a lot of the writing stops being so literal. My guess is it's the pain, both physical and mental - and later on, the morphling used to numb the physical pain that are the reasons for this. There are moments, before and after she was given morphling, when Katniss does seem to be hallucinating.
Katniss did talk about sea water, trying to breathe deep under the water's surface, though that may have been the feeling of breathing through lungs temporarily damaged by, presumably, smoke inhalation.
